A Refundable Proactive Coaching Investment is billed after your follow up Carotid Intima Media Thickness ultrasound and lab results (4-8 months) as follows:
$350/year/individual (Not payable by third parties CPT code 89990)
$550/couple billed as $275 per individual (Not payable by third parties CPT code 89989)
This is an investment in your ongoing success. It is billed after and while you have enough experience to understand the value and compare it to other experiences. If you think it’s too much, check the cost of stents, surgery, rescue and rehab after a stroke or heart attack and other similar options.
It is billed by text invoice, payable by credit card or check. “Nonbillable or nonpayable” (which really means not recognized by third party payers as payable) code is 89990 (individual) or 89996 (half of a couple). It is not submittable to third parties for payment because it is payment for noncovered benefits and service. It is your personal investment in the benefits. Payment is appreciated, as much as you can as soon as you can.

If you don’t want to pay it , if you don’t think it is reasonable, you are welcome to pursue the standard care experience and results with widely available alternatives.
If anyone you refer successfully starts a CURE Plan, $100 will be credited toward your next annual fee. Please email drbacks@thecurecenter.life to make me aware of the referral.
Finally, if you suffer a stroke or heart attack in spite of following our Proactive Medicine care, the last paid fee will be refunded, no questions asked.
